We look forward to the opportunity to help you achieve optimal function. Before your first visit to our clinic, we have attached some easy forms for you to print and fill out prior to your visit. You can also email us. If you are unable to do so, please arrive 15 minutes prior to your scheduled appointment time.

Below is an abbreviated list of some of the insurances with which we are In-Network. We can also verify if your insurance has Out-of-Network benefits.

If you are “Out of Network”, many times your insurance may reimburse you for some or all of your PT services, especially if you have a PPO.

Frequently Asked Questions:

Q: What do I bring to my first visit?

A: We require photo ID (driver’s license) and your most recent insurance card. Upon arrival a copy of both cards will be made for our records. Please also bring and referrals or prescriptions that were given to you by your doctor.

Q: What do I wear? 

A: You will be moving around quite a bit, so we suggest wearing something comfortable. Exercise attire (sweatpants, shorts, t-shirt) is best. If you aren’t able to bring the suggested attire, we can work around it.

Q: Do I need a referral or prescription from my doctor for physical therapy?

A: In most cases, you do not need to see your doctor prior to coming to PT. Often, this can save time and money and help you get on the road to recovery faster. We are trained to recognize when you need to see a physican and there may be times that we recommend you see your doctor, as needed. Our staff will verify with your insurance whether you need a referral or not. (Medicare is one insurance that always requires a prescription).

Q: What should I expect on my first visit?

A: Once you complete your initial paperwork, the physical therapist will review your medical history. The initial examination process generally takes about an hour and will include a thorough discussion of your problems,  several diagnostic tests/movements to assess strength, flexibility, pain, balance, and the way that you move

Q: Do I have to have an injury to come to PT?

A: No! Physical therapists are movement specialists and are experts in the musculoskeletal system. We are able to detect movement impairments and imbalances that may be contributing to a current injury or placing you at risk for a future injury. We also provide therapy to address these imbalances and decrease your risk of injury while improving your performance.  Please see our evaluations and programs pages for more details.

Q: How do I know what my insurance benefits are?

A: Upon receiving your insurance information, our staff will verify your benefits for PT and notify you of your responsibilities before treatment begins. However, nothing quoted to our staff is a guarantee of coverage or payment.

Q: What forms of payment do you accept?

A: For your convenience, we accept cash, credit card, and personal checks.
